Yahoo may not be able to import notes, but you can do it without anything else:

1. Go into Palm Desktop and Select Memos
2. Select File > Export
3. Click All under Range.
4. Select Tab Separated Values from the Export Type drop-down list.
5. Browse to the folder where you want to save the file.
6. Give it a file name followed by .txt (example: notes.txt)
7. Click OK on the Speciality Export Fields window.
8. Click OK on the Export Complete window.

Now open / go into Outlook.
1. Select Notes.
2. Select File > Import and Export.
3. Select Import from another program or file > Next.
4. Select Tab Separated Values (Windows) > Next.
5. Browse to find the file you saved in the first steps > Pick whatever you prefer in options > Next (it takes a little time to import).
6. Select Notes as the destination folder, if not already selected > Next.
7. Ensure the checkbox is selected by "Import..." > Map Custom Fields.
8. Match up From to To: fields. Typically, first one is Notes Body, second is Sensitivity, and last one is Categories.
9. You can click Next to see how other records import (optional).
10. Click OK > Finish.

Have not figured out a way to import PalmOS To Dos into Outlook's Tasks.

On the mac, Palm desktop 4.2.1 exports files to .vcs (vcal files) and you can import them into outlook on the pc. However it doesn't import recurring events. This worked smooth.
